Heyo!

I had surgery to remove a pilonidal cyst about 6 weeks ago. It was the surgery with skin flaps by a plastic surgeon. He said after the stitches came out that I should take it easy for a couple of weeks (i.e. no running).

It's been about four weeks since then and although I can walk and do most things normally I still experience a fair bit of discomfort when I move any faster than a brisk walk or when I try to jump up and down, even when I lean back too far in my chair. I tried to have a look at the wound in the mirror and it sort of looks a bit different and the top than the bottom.

Is this normal? I'm a bit concerned because I usually dance a couple of times a week and I haven't been able to since all of this

Hello

I had my surgery for pilonidal sinus at the beginning of March 2010. It went well (that was the doctors opinion at least), the surgeon used the open wound technique. So the healing took about one month.
The problem I am facing now is bleeding from the same area that the sinus used to leak before the surgery. It looks like a blister, and it is really close to the anus. That is probably why it wasn't removed during surgery.
I have experienced bleeding after almost every long drive I had since the surgery.
I have to mention that the bleeding takes place only after long periods of sitting down. Especially after long drives with the car.
I followed the tips on the website for keeping the wound clean and dry. I used also two types of healing ointment.
But the "blister" just won't go away...

Has anyone else experienced something like this? Any tips on how to overcome this?

Hey there. I just had a pilonidal cystectomy at about 9 AM on Tuesday, July 6th. It is now 2 AM on Thursday, July 8th. I noticed, after laying on my couch for a few hours and having gotten up for a minute that my couch had a few new red stains, and promptly checked my bandages. They are completely soaked with blood, and my boxers were soaked, along with my gym shorts that I've been wearing all day. Is it normal to be bleeding fairly heavily the day after this surgery?

I called the hospital and spoke with the doctor on-call, and she told me to just keep pressure on the area, but not to change the dressing. I wouldn't say that I'm bleeding "profusely," but I feel like it's too much. The whole area seems a lot more sore than it did just four hours ago, and the percocet they gave me doesn't seem to be working as well as it did just after the surgery and throughout most of the past day. I'm a little worried, and intend to make my way to the hospital before noon today.

Has anybody else had this problem? Does this sound abnormal? Should I change the dressing, or listen to the docs when they say to leave it alone? It seems like a bad idea to leave blood-soaked bandages on a wound for very long...

Hi All,

First of all, a big thank you for the administrator of the site to have such a website dedicated to little unknown disease.

I need some help to identify what exactly i have. I first noticed a bump at the fag-end of the tailbone/coccyx area in 2006. I used to travel around 4 hours in the car to work on bumpy roads. I had severe pain in the tail bone area for around 3 months and the bump disappeared in 3 months and I never had a pain again

However, I have had these bumps reoccur every year for a span of 3 months. I later found out from the internet that it could be PS. I went to the doc 2 years ago, he gave me some anti-inflammation drugs and I was all well again. The bump disappeared and so did my pain.

I've just got the bump recur now and its painful as hell again. There is no redness or oozing from the bump. It was huge 2 days ago but i took adequate rest and the size of the bump as halved now. There is still pain but i am managing it.

I just want to know whether this is PS and should I have a surgery ASAP. Going by my history with PS, the bump comes and goes! I was too stressed in the last one week and i am pretty sure the stress was the reason for the bump to be back.

Any advice on what I should do now? Thanks a ton in advance

Hello

Im yet another unfortunate one

Last year in Feb I got a bit of pain at the top of my ass, when to the doctor and he gave me so antibiotics, two days later I could not walk, talk or anything, so much pain it makes me cry thinking about it lol.

The doctor performed minor surgery, basically lanced the barsteward while given local anesthetic. This then involved 4 weeks of daily trips to the doctors/hospital to get it packed and cleaned etc. By end March I was all hunky dory.

From then until a few weeks ago all has been fine, although I do get a bit of back ache if I sit for long periods in that area.

Two weeks ago I noticed a little bump, not painful. I had some salt water baths as I do 2/3 times a week as the nurse told me too last year.

This seems to work but to my horror the next day it looked like a bruise and was painful.

I went to minor injury at hospital as it was the weekend. The practice nurse gave me antibiotics and said to get my doctor to refer me to surgery asap. I went to the doctors 3 days later as it had not gone. He refused, saying to give it another week or two of tablets.

I told him that I have read up about this and it needs surgery and that doctors keep fobbing people off with pills that do NOTHING. He still refused saying it needs to flare up like 10 times in a year for the NHS to do surgery on it.

So I went back to minor injuries at the hospital and told them. They could not believe it and they rang the surgeon at the hospital straight away and I got booked in the next day.

To my horror I only knew 5 mins before being pushed down the corridor on a bed that they were giving me general anesthetic

10 hours later I went home, very sore. Now every day I get a district nurse to my house to clean and pack it. They say that they must have missed some infection last year thats why it is back, yet the nurses last year said about how clean and good it healed.

Its a week since my surgery, the nurse said "its strange as the surgeon cut a diamond shape hole and its only 1.5 cm deep and 1cm long, a very small abscess.

This worries me as last time it was 3 cms deep and 2.5 long. If they didnt get it all last time how is half the size this time different?

The surgeons notes say they "put his finger in the wound" and "all infection was cleaned out" etc.

I dont know what to do as my work is nearly on the verge of sacking me due to this sickness record for this bloody problem.

I am also longing to sit back in a arm chair and stretch my back like the good old days, as I lay on my right as its the most comfortable, my right arm now hurts when i move it it never ends.

Would this surgery that I just had mean that its crap and it will come back?

I work for the Police so I wear a half a stone utility belt on the exact area and sweat alot due to the stab vest. Although I bath in salt water most nights and shower afterwards every time and keep the area hair free.

Any ideas?

I am not a naturally hairy person, the abscess is at the top of my bum crack and goes up the bottom of my back. I have never had hair there or on my back. I have regular salt baths with natural sea salt and shower afterwards and use a scrunchy to exfoliate the area. Have done since last year. I don't sit down a lot and I am a little over weight.

Hi all.

This is the story of my pilonidal sinus, however it is mainly about the issues I've experienced since having the sinus removed.

I had a recurring bump in the "region" for around six months before I went to the doctors to have it looked at. Once I went to the doctors (just under four years ago), they diagnosed it as a pilonidal sinus. Options weren’t really discussed, just that it had to be removed by an excision.

When it came to the operation (06/10/2006), again the options weren’t really discussed, they just knocked me out and did it. When I came round after the operation, I was quite surprised; the sinus had appeared only to be on one buttock, however the excision had been performed on both sides, going right down into the cleft of the buttocks and up the other sides. The wound had been closed and stitched – a technique called “marsupialisation” I was told afterwards. It had been stitched on each buttock individually, with the stitches being down the central cleft.

I had a lot of pain on returning home, which I knew was to be expected. Unfortunately, one night in bed, one side of the stitching burst, which then left the wound open. The practice nurse came to see it the next day and sent me to A&E to have it examined. At that point, they said it was too late to stitch it back down, and so it was left open. District nurses came to my home to clean and pack the wound daily thereafter, however over the next few months I had numerous infections, and the pain never went away. The wound took over 3 months to heal completely.

The wound is now fully healed, however the pain still remains. I have been to see numerous consultants and pain management specialists about the pain, and they have yet to offer a reason for it. I have had an MRI scan (with no staining) which revealed a small amount of scar tissue around the area, but again nothing that would account for the pain I experience on a day to day basis.

Currently I am unable to sit down for longer than around 10 minutes without the pain manifesting itself, and I have tried sitting in every conceivable position. If I am forced to sit for a long period (for instance on a journey), the pain becomes really severe and persists for days afterwards, leading to extended periods of absence from work, which is obviously causing issues. I’m also unable to lay on my back and unable to bend much, which means many activities are out of the question.

So far, the treatments I’ve tried are: Co-Codamol, anaesthetic patches applied directly to the area, acupuncture, steroid injections, pregabalin (nerve inhibitor) and hyaluronidase injections to try and disperse the scar tissue.

Yesterday I went to see a different consultant at a private hospital to see whether they could offer another opinion, however they basically said they couldn’t think of a reason for the pain and suggested trying amitriptaline and and a steroid cordal injection.

Does anyone else here have similar experiences, and if so, is there any advice on offer for a possible long term solution to the pain? Even short term would do, as long as it meant I can start to do the everyday activities that have been denied to me for the last four years! One thing that was mentioned sometime last year was that my coccyx could now be closer to the surface than it used to be (ie. less padding) and the pain could be linked to that? Does anybody have any ideas on how to deal with such a situation?

Many thanks all, and apologies for the essay!

I have browsed this site before, and I must say Steven, you have done a wonderful job in providing advice, support and hope to the people who visit here. I am posting today to tell my story and note some observations I have found.

I am now 35 (born August 1974) and suffered a pilonidal cyst in Dec 1995. Like may of the posters on here, started as a fluctuant lump, progressing over the space of 5 days or so to the point i couldnt sit/stand/breathe/eat or anything! Went to pharmacy, and they thought it was a bruise! Eventually saw GP who diagnosed PS straight away, and i was admitted that day for an emergency incision and drainage. Had the operation and the relief was unbelievable, however the procedure was barbaric to say the least. I had a gash at the top of my crack, about 5cm long, 1.5 cm wide and about 4 cm deep. After the operation (left open to heal by secondary intention) I was in hospital for another 2 days. During this time i was left on a bed caked with blood, not once was i offered a sheet change of painkillers! Left hospital and had district nurse come and pack the wound for approx 4 months. Seems like wound was healing well.

Had follow up at hospital, when the bombshell was dropped that i had developed a sinus. Apparently 50-65% of people who have I & D procedure go on to develop the sinus. I was horrifed, considering the pain the last operation had caused. So I didnt go back............

wore gause dressing/dressing pad for in total 11 months, each time the wound got close to healing, i would sit on the toilet to defacate and the wound would break open and pour with blood. Also a browny discharge with a foul smelling cabbage like smell. Still i refused to go back and did NOT seek any further treatment.

The wound ventually healed, if you look at my bum crack now, at the top of the crack is approx a 5cm scar, however on top of the scar tissue i can see like a tube. I assume this is the sinus. the two holes i had at the bottom of the sinus closed. Now just looks like a normal scar

So here we are now, nearly 15 years on............. Sinus still in place. I can honestly say, after the first 12 months it took to heal over I have had no problems whatsoever (touch wood). There has never been any swelling/discharge/pain or anything. I also have a job that involves sitting all day. Im not saying it will never come back, but hopefully as im nearly in the 2nd half of my 30s, and it has been 15 years since my problems, then i may be able to live with the sinus still attached to me lol.

So it just goes to show that surgery may not always be the be all and end all. I have lived with this after developing a sinus for nearly FIFTEEN years with no trouble at all, and have no flare ups, nothing.

I have noticed a few things (most of which im sure you already know) which seem to be prevalent in people with PS

Most people affected seem to be dark haired males, with course or curly hair.

Seems to mainly affect between years of 18 - 25

Obesity/being overweight does not help the situation.

I would be interested in learning how many sufferers are cigarette smokers (i wasnt when i was diagnosed but have been for the last 6 years)

My surgeon told me that some people are susceptible who had a dimple in the skin near the coccyx bone

Mine started after sitting down hard on a peice of wood that was inside the sofa! Trauma seems another major factor.

I have gone on to have problems with abscesses in groin, and also carbuncle/boils inside thigh, again caused by course hair.

Anyway, enough of me waffling, wanted to say what a good job youve done, and to all you sufferers out there i wish you nothing but good health.

Best of luck

I'm an unlucky long term sufferer of a recurring pilonidal sinus. I've had four operations in just over 18 months, including two primary closures, two secondary closure procedures and phases of negative pressure wound therapy. My last operation was in mid-December and since then I have gradually managed to heal up to the best conditions that I have experienced during this saga.

I'm well familiar with my wound and keep it very well cleaned, hair free and under as little pressure as possible to give it the best chance of regeneration. Within the last seven days, I have noticed some incredible healing since I have taken the decision to stop using my Sorbsan and Aquacel dressings as the wound was no longer large enough to require them. I have a thick ring of epithelial tissue around what's left of the wound and am in no real pain or discomfort to speak of.

What I have noticed though is a collection of thick discharge (think the consistency of say mucus) from the wound. It isn't a large amount and doesn't have the stench of infection, instead it just smells bodily and of sweat and is generally a very pale yellow colour. I was surprised because the wound is now quite shallow and was thinking that it could be exudate. As far as I can tell, this isn't drainage from a sinus, I've thoroughly checked the wound and have had a tissue viability nurse with good experience agree that there are no tracts present. Does anyone know what this is? I also shower and take two salt baths daily and ensure that the wound is well dried afterwards.

Any help would be appreciated.

A really friendly consultant who talked me through the process clearly and concisely every step of the way. Surgery appears to have been a complete success and there was very little in the way of pain following it. Looking at the wound itself, its very clean and nicely done to the point of which i'm almost quite proud of it

I had surgery for my pilonidal sinus done at the end of january, procedure went extremely well. Sinus was excised and then the whole thing has been sealed over, not sure the right term for it but basically took a flap of sking so that the buttock cleft has been sealed at the top half above the anus.

Stitches came out and all is fine but I have noticed a lack of sensation and sometimes swelling around the wound. I assume this is really just a case of the long healing process and that with exertion and such it will swell up from time to time. Just thought I would ask about similar experiences....

Just joined the forum after reading it for several weeks.

I had my op for a Pilonidal Sinus back in early December 2009 and after 2 infections and more recently an abscess, the wound has stopped healing.

My surgeon referred me a Tissue Viabilty Nurse who has been brilliant, very positive and supportive. She advised that the wound can be healed is superficial, so good news at last!!

However, my problem has been my perceived lack of support from my GPs.

Since December I've been to see various doctors on numerous occasions and on some visits they have even failed to examine me, I just received a sick note from work instead.

On another occasion. the GP told me she couldn't see a scar let alone a wound and despite me needing help to stand up, was willing to sign me off to return to work!

The Tissue Viability Nurse called my GP surgery and gave them clear instructions as to what needed to be done by the nurses when cleaning and re-dressing the wound.

One of the tasks was the shave the area which is incredibly difficult to do yourself.

One of the nurses was so rough with the razor that she had to clean blood off it afterwards. Another decided after a minute she would leave it for another colleague to do instead, so 2 weeks after seeing the TVN, the area still needs to be shaved properly!

They at least arranged for me to visit my local walk in centre for the re-dressings to be done on Sunday, at which I was advising the nurse how to apply the dressing.

On Monday morning I woke up in a mess no details needed) and after calling my GP for an appointment I was told I'd have to wait until after 5pm. This meant waiting all day, cleaning the area, changing clothes etc.

Eventually got to see the GP and nurse after a very long delay and it seems I now have a burst abcess.

I noticed that someone else posted a message to say that shaving can aggrivate the area and cause an abcess, can this this be confirmed?

I returned to the TVN yesterday who was again briliant, she cleaned and dressed the wound properly, (no dressings falling off after an hour), but she's spoken with the surgeon who wants to see me next week.

Anybody kow what he's likely to say? Or is it just to check on the wound, post abcess, which feels better today, thanks in part to the Metronizadrole (spelling needed). So hopefully, by next week, it should be clear.

The TVN did say the wound tissue is clean and healthy and wants to continue with the AG dressings she's been using, but now I'm worried that the surgeon is going to insist on a second op.

Apart from this abcess, I felt much better using the proper dressings and thought I was making progress.

I just feel that the general level of service my from local GP surgery has been poor and some of these problems could have been avoided.

Hi!

I had a pilodinal cyst around 2 years ago and I went to the doctors who diagnosed it and it eventually drained on it's own.

However, over the last year I have been getting the pain every couple of months (hard to sit, feels hot, etc) but I think the abcess must be deep as I cannot feel a lump and it never bursts.

Around the same time of these episodes I have a day or two where I feel terrible, a bit like flu but when the cyst starts to hurt I feel better.

What I wondered is whether it is worth going to the doctors with it again, is there much they can do?

Hope you can help!

I am a fit, youthful 58 year old man, living in Bangkok Thailand, who was diagnosed with a chronic pilonidal sinus and immediately underwent surgery for its extraction ten days ago. Just two ago, the doctor took out 10 of 12 stitches, however, the day after, yesterday, I rushed back to the hospital in pain and dropping blood apparently caused by too much tension on the remaining stitches. The doctor removed those and I have felt better since. However, I now see an open wound ( I guess normal?),there's some oozing and i stink down there. Anyway, I'm advised just to wash, shower and bath in water with no soap and wear a sanitary napkin. I visit the doctor again next week. Today, I just lounged around in bed and on the sofa wondering do I need to know more, am I doing the right thing, should I get another doctor's opinion...bla, bla, bla. Any words of advise?

I have found Hydrocortisone Cream to be an effective treatment for skin soreness of pilonidal wounds. It is a very safe cream to use with wounds as it is often recommended by GPs. It is available both on the high street and by prescription.
